Medical Authorization Form
Due to state law, if your child has a medical condition, such as asthma, seizures or diabetes, that could be life-threatening, your child will not be able to start school this fall unless all required forms are turned in by September 1. Please visit our website for more information and to find the form. If your child has a life-threatening medical condition, you are also required to have an appointment with your school nurse before the first day of school, in person or by phone. Please call your child’s school for the nurse’s schedule and plan to complete all the necessary steps so your child can start school on time.
We understand that medical insurance can be costly and sometimes difficult to obtain. In support of our students and families, we have partnered with Myers-Stevens & Toohey, a national student medical insurance administrator, to offer all families access to a low cost, voluntary student accident and sickness insurance program. Details of the coverage options can be found in the digital brochure on our district website.
We will hold an informational webinar for families on Monday, August 30 from 5:00 to 6:00 p.m. This session will provide information about the 2021-22 school year.
