Due to state law, if your child has a medical condition such as asthma, seizures or diabetes that could be life-threatening, your child will not be able to start school this fall unless all required forms are turned in BEFORE the first day of school.
You must provide your child’s school with a medical authorization form completed by a doctor and signed by a parent; all medications and equipment your child needs; and an emergency care plan, which you should develop with your school nurse. You are also required to have an appointment with your school nurse before the first day of school, in person or by phone. Please call your child’s school for the nurse’s schedule and plan to complete all the necessary steps so your child can start school on time.
School begins on September 2.
