If your child in grades 6-12 needs over-the-counter or nonprescription medications, Washington State now requires a licensed healthcare provider to sign a Medication Authorization Form (MAF). In the past, the form was not required for secondary students. Now MAFs are required for all students for OTC, nonprescription and prescription drugs.

In addition to the healthcare provider's signature, MAFs will also be signed by the parent or guardian. Forms will be approved by the school nurse.
Nonprescription sunscreen may be carried and self-administered at school without healthcare provider authorization or a MAF on record.
